Looking to obtain a copy of your medical records?

We offer two ways to access your medical record:

MyHealthOne Portal:

MyHealthOne consolidates many common tasks into one secure, easy-to-use online patient portal. It gives you access to most of your medical records on your desktop computer, laptop, tablet or smartphone 24 hours a day.

*Note that some medical records may only be available through our hospital Medical Records office.

Paper Copy of Medical Records:

You can obtain a paper copy of your medical records by completing the Medical Records Request form and faxing it to the Health Information Managment office at the hospital where your child received care.

Please note that birth and death certificates are processed through the Colorado Department of Health Vital Statistics Department.

Medical Records Costs and Timelines

Depending on the request, fees may apply. Please allow a minimum of 10 business days for processing time. Records for physician office visits will be handled urgently to ensure continuity of care.
